What is Total Thyroidectomy?
Total thyroidectomy refers to the surgical removal of the entire thyroid gland and part of the isthmus, the connection between the two lobes. Any disbalance in the production of hormones or change in the appearance of glands raises the need for a total thyroidectomy. Your doctor may also recommend total thyroidectomy instead of hemithyroidectomy or partial thyroidectomy if both thyroid lobes are infected or there is any risk of recurrence.
Total Thyroidectomy Surgery
This is performed to eradicate symptomatic or cancerous nodules of the thyroid gland, remove goiters, or treat symptoms of hyperthyroidism (a hyperfunctioning thyroid). This surgery is performed together by the surgeon and endocrinologist, who treat your body’s hormone imbalance. It is performed under general anesthesia, and an incision is made in the lower central neck to remove one of the two thyroid lobules. It takes between 2 to 4 hours, depending on how much thyroid tissue the surgeon removes.
Causes & Symptoms of Disordered Thyroid Glands
Total thyroidectomy can be performed if your body has an autoimmune problem, too much or less iodine in the diet, a tumour (cancerous or non-cancerous) in the pituitary gland, excessive consumption of thyroid medication, a swelling in the thyroid gland (goiter), or an inflammatory process.
Symptoms
- Difficulty in breathing or swallowing
- Large nodule or goiter
- Hyperthyroidism (overactive thyroid)
- Weight loss or sweatiness
Treatment of Thyroid Gland
Before the doctor starts the surgery process, they will order imaging tests or an ultrasound/ct scan. It helps in diagnosing the type of disorder and size of the thyroid lobule to be removed.
To access the thyroid gland, one of the neck's skin creases is dissected. The surgery is performed under general anesthesia, which means you’ll be asleep during the procedure. Your surgeon will make an incision (usually 2-3 inch in length) to remove entire thyroid lobules. It is necessary to have a surgical drain through a catheter (a small tube), which is inserted for at least one night after surgery. The surgical drain can typically be removed on the second postoperative day after the draining has stopped. Once your surgery is done, the incision will be closed with stitches (sutures).
Advantages
- Balanced thyroid hormones
- Relieves compression
- Lower risk of hyperthyroidism and hypocalcemia
- Better muscle and bone health
Challenges
- Bleeding and infection
- Sore throat
- Voice change
- Injury to the esophagus or other nerves
- Hypothyroidism (low thyroid hormone)
Cost of Total Thyroidectomy
Total thyroidectomy surgery can be very expensive as it involves the treatment of very delicate organs with a high risk of infection. The cost of total thyroidectomy surgery in India starts at 1,20,000 INR and can go up to 1,80,000 INR. The cost can differ due to several factors, such as high-tech instruments, the doctor’s fee, the size of infected lobules, patients’ complications, and many others.
