What is Parotid Gland?
The parotid gland is the largest salivary gland and is located just in front of the ears. Tumors in the parotid gland are caused by the abnormal growth of tumours or cells in the parotid gland.
There are two types of parotid gland tumors:
A superficial parotid gland tumour develops over the facial nerve and is confined to the superficial lobe of the parotid gland.
A deep lobe or total parotid gland tumour develops under the facial nerve and is confined to both lobes.
Parotid Gland Surgery
Parotid glands are so common, yet the surgery to treat the condition is challenging as facial nerves must be protected during the procedure. So, the main aim is to remove the entire tumour without harming the "facial nerve." Parotid gland surgery is also known as parotidectomy, which is a surgical procedure to treat or remove the parotid gland.
Causes & Symptoms of Parotid Gland Tumour
There are several factors that can affect your parotid gland such as viral infection, poor oral hygiene, salivary duct blockage, HIV, dehydration or malnutrition, and many other health problems.
Symptoms
- A lump or swelling near your jaw
- Discomfort while swallowing
- Pain near the area of the salivary gland
- Dry mouth
- Numbness or muscle weakness in part of your face
- Difficulty in opening a wide mouth
- Pus and foul-tasting drainage into the mouth
- Fever and pain
Parotidectomy Treatment Procedure
Doctors use surgery to access the gland and remove a parotid gland tumour, also known as a parotidectomy. In this procedure, they either remove the infected part or remove the whole parotid gland.
Laparoscopic
Using the minimally invasive technique, a small incision is made in front of the ear and down the neck. The surgeon uses a nerve monitor to find the facial nerve, which needs to be protected throughout the procedure. They will separate your facial nerve and its branches from the parotid gland tumour. Your surgeon removes the parotid gland tumour in a biopsy if it contains cancer cells. In the end, the incision will be closed through sutures (stitches under the skin, giving an excellent cosmetic result).
Chemotherapy
If the patient is not a candidate for surgery, then doctors also use radiation therapy or chemotherapy to kill the cancerous cells through powerful radiation beams or medications, respectively.
Advantages
- The procedure is beneficial to all age groups; however, older patients with various age-related complications benefit the most.
- Low rate of surgical morbidity
- No scarring
- Lesser facial nerve damage risk
- Less pain and swelling
- Same day discharge
- Rapid recovery
Challenges
- Due to bruising and swelling during surgery, the tongue gets numb
- Salivary gland surgery can damage the facial nerve
- Frey syndrome (sweating over the face while eating)
- Lip movement difficulty
- Difficulty in chewing or swallowing
- Infections or bleeding
- Scarring
Cost of Parotid Gland Surgery
The average cost of parotid gland surgery in India can be approximately Rs. 1,00,000 including hospitalisation and medications. The cost varies depending on the size of the tumor, the affected gland, the patient's co-morbidities, and a variety of other factors.
