What is Pilonidal Sinus?
The pilonidal sinus is a small skin hole or pore that appears at the top of the buttock. If this sinus is left unnoticed, it may get infected and fill with pus or an abscess, forming a pilonidal cyst. This cyst contains dirt, hair follicles, debris, dead cells, and pus. It is an acquired condition in which hair penetrates the skin of the natal cleft due to buttock motions.
Pilonidal Sinus Surgery
Although the pilonidal sinus is not life-threatening, it does hinder daily activity, causing pain and discomfort. The acute or dormant pilonidal sinus remains unnoticeable and does not require surgery unless it gets infected and shows symptoms.
If there is discomfort or discharge from the sinus, surgical treatment of the pilonidal sinus is most likely required. This includes the drainage of the pus and closure of the infected sinus with no chance of recurrence.
Causes & Symptoms of Pilonidal Sinus
The most common cause of pilonidal sinus is a sedentary lifestyle, i.e., sitting for long periods of time or having little physical activity. They are more common in males aged 20–30 years old. Other factors causing pilonidal sinus may include changes in hormones, hair growth (especially coarse and curly), friction from clothing, etc.

Treatment of Pilonidal Sinus
Incision and Drainage
They are the most common procedures for treating the pilonidal sinus. It is done under local anesthesia, and a small hole is made in the cyst to drain the pus.
Pilonidal Cystectomy
This is a broad excision procedure that involves the removal of the cyst as well as surrounding tissue. This procedure is used if the cyst is large and there is a risk of it recurring.
Laser Pilonidoplasty
In this procedure, the surgeon uses radial laser fibre to ablate the sinus tract and drain pus from the cyst. This treatment procedure is done with 1-3 small incisions in the abscess with the help of a laser beam. The surgeon then closes the wound, ensuring that it does not close prematurely (before full drainage of the pus). Laser treatment of the pilonidal sinus helps heal the wound faster with a low risk of recurrence.
