What is Anal Fissure?
An anal fissure is a small tear or cut in the inner lining of the anal (mucosa) or sphincter muscle. They mostly occur due to the passing of hard or large stools, affecting both genders equally at any age. According to their severity, they are of two types, i.e., acute anal fissure (fissure for less than 6 weeks) and chronic anal fissure (fissure for more than 6 weeks).
Anal Fissure Surgery
If you experience bleeding or pain during bowel movements, then your doctor or proctologist may diagnose and recommend surgery for an anal fissure. Anal fissure surgery is the treatment used to eliminate this anal fissure by cutting out the fibrotic tissue or sentinel skin tags. Most doctors recommend laser surgery for fissures, or laser fissurectomy, which uses a laser beam to make an incision and destroy scar tissue and loosens the internal sphincter to release tension and reduce pain.
Causes & Symptoms of Fissure
There are several reasons causing anal fissures, of which the passing of hard or large stool is the main cause. Diarrhea, iatrogenic (any previous surgery insertion), childbirth, anal sex, poor diet (low-fiber foods), Chron's disease, or injury can all cause an anal fissure.
Symptoms:
- Painful and bloody defecation
- Sentinel pile (a skin tag-like growth or small lump near the anus)
- Itching and burn on sphincter muscle
- Visible cracks around the anus
Treatment for Fissure
Surgery might not always be needed to treat an anal fissure, and it may heal on its own. If the fissure is acute and harmless, then your doctor will recommend certain stool softener medicines and make some changes in your diet (adding fiber-rich foods) and some local ointments. If the fissure is chronic or non-surgical treatment is ineffective, your doctor will diagnose the problem and recommend the best course of action. The treatment for fissure involves administering saddle anesthesia before proceeding with the subsequent surgical procedures.
Lateral Internal Sphincterotomy
This procedure is an open surgery where the surgeon uses a scalpel (a sharp knife) to cut the fibrotic tissue or sentinel skin tags. It aids in releasing tension, reduces pain and spasm, and relaxes the sphincter muscle.
Laser Sphincterotomy
This procedure is a painless, minimally invasive method with no cuts or stitches. In fissure laser surgery, the surgeon uses a laser beam to make an incision and destroy scarred tissue and cut internal sphincter to release tension and reduce pain without any blood loss.
Advantages
- Relax the sphincter muscle
- Minimally invasive
- No stitches and bleeding
- Lesser operative time
- Quick recovery
- Relief from fissure pain
Challenges
- Infection and bleeding
- Difficulty urinating
- Development of an anal fistula
- Bowel movement difficulty (2–3 days after surgery)
- Pain and brushing
