What is Phyllodes Tumor?
Phyllodes tumors of the breast are a rare tumour type that occurs mostly in women's breasts. They are firm, painless breast lumps that can be benign, malignant, or borderline—a condition between benign and malignant.
Phyllodes in Greek means "leaflike," which says that the tumour takes on a leaflike appearance under the microscope. These tumors are typically benign (non-cancerous) and develop in connective tissue (stroma) rather than ducts or glands, where tumors normally develop.
Phyllodes Tumour Surgery
Phyllodes tumours are sometimes confused with fibroadenomas. But unlike fibroadenomas, which are non-cancerous and grow very slowly, phyllodes tumours grow very quickly in the short term and can also stretch the breast skin or cause an ulcer on the breast. Therefore, phyllodes tumour removal is mandatory before it attacks the whole breast and spreads cancer to other parts of the body.
Causes & Symptoms of Phyllodes Tumors
In most cases, the cause is unknown, but women with a rare inherited genetic disease called Li-Fraumeni syndrome are more likely to develop phyllodes tumours. It can also develop as a result of an injury, breastfeeding, pregnancy, or an imbalance of the hormone estrogen. It mostly occurs in women aged 35–55 and rarely develops in males, accounting for less than 1% of all breast cancers.

- Red or grey-yellow coloration of the breast
- Bloody nipple discharge and painful (rarely)
Treatment of Phyllodes Tumors
The treatment of the phyllodes tumours may include the complete removal of the tumour along with the breast tissues. A diagnosis can often be made with a mammogram, ultrasound, or MRI scan. Sometimes the entire tumour may need to be excised to determine whether a tumour is a phyllodes tumour and whether it is malignant or not. Depending on the size and severity of the tumor, there are three surgical treatments available to remove phyllodes tumors:
Wide Excision
If the tumour is benign, an excisional biopsy is all that is needed. In this surgical procedure, the entire tumour is removed and sent to the lab for further examination.
Lumpectomy (or partial mastectomy)
The excision of a malignant or borderline breast tumour usually includes the removal of the mass itself as well as a small quantity of the surrounding healthy breast tissues. The phyllodes tumours are removed by making a small cut near the lump and sealing the wound with stitches.
Total mastectomy
If the tumour is not removed at its early stage and spreads to the whole breast, then mastectomy is required. In this procedure, your surgeon will remove the entire breast. Breast reconstruction can be done concurrently with or after your mastectomy.
Your surgeon may also use radiotherapy or chemotherapy to kill cancer cells (if any have spread to other parts of the body).
